Creating or Resetting Your Provider Portal Password (Walkthrough)
To simplify the provider experience, we've designed the process for creating and resetting your Provider Portal password to be the same! Below are the steps that outline this process. We've also created a quick walkthrough video that demos this process:
To get started creating or resetting your Provider Portal password, please follow the steps below:
1. Visit the link - https://provider.lyrahealth.com/login. You are welcome to bookmark this login page to save into your browser for future use.
2. Select "Forgot Password?".
3. You will then be directed to enter your login email address:
4. You will then receive an email which prompts you to Reset your password by clicking the teal button:
5. You will then be redirected to a page where you will enter your new password:
Please note that your password must meet the following criteria:

Your password must be at least 8 characters long

Must contain at least one upper case letter

Must contain at least one lower case letter
-
Must contain at least one number (0-9)

Does not contain any part of your username

Does not contain your first name
-
Does not contain your last name
Once you created your new password, click Reset Password.
6. You will be directed to a page that provides confirmation that your new password has been set with a banner at the top stating: "Your password has been successfully changed". Take a moment to note the URL of this page, it will most likely list Care.Lyrahealth.com, you will need to redirect yourself to the provider portal login page listed as Provider.Lyrahealth.com. Please click this link directly to be directed to the Provider Portal Login page: https://provider.lyrahealth.com/login.
7. You're all set! Sign into the Provider Portal with your new password!
